For #21: Here, as with pretty much all resolve questions, you need to find an answer that provides a cause for the discrepancy. The discrepancy is that even though it was determined that fatigue was from too little sleep, the patient was not told to sleep more. B does not provide a reason why the patient would not be advised to sleep more. I see how someone could think that if the first two hours are the most important then maybe more sleep wouldn't matter, but that actually contradicts what you are told in the stimulus: the patient needs more sleep. E gives a reason why more sleep was not advised: knowing you need to sleep more can cause you to sleep even less due to worry.
