I'm glad to give you my take on your situation:

As to whether you should mention if it was OCD, or simply mention it as a "medical condition," the choice is really up to you and whether you feel comfortable disclosing the name of your illness. Typically, I advocate in the name of full disclosure in addenda, but when it comes to medical conditions I tell students to name their condition only if they feel comfortable doing so.

As regards your second question, I would definitely include an addendum explaining the jump in your GPA as being related to receiving treatment, even if the school doesn't ask for one. When it comes to transcript interpretation, giving schools as much information as possible to make an informed decision is definitely the right step.
