What should the format of an LOCI be? In terms of spacing and the like.

Make it just like a regular business letter. This is a professional correspondence, so make it look as pro-style as you can.

So single spaced with no indentations and one space between paragraphs? All in an attachment to an email?

That's fine. The exact specifications of this (single spaced, double spaced, etc) aren't going to make a material difference
