Transcript Request Help

Hi.

I have a question about requesting transcripts.

My school does their form electronically,

And I have registered for CAS, but have not yet taken the LSAT.

I am trying to request my transcripts from my Master's degree program, but am not having any luck since i cannot find the lsac transcript request form. Can someone please help!

Also, I have an official copy of my undergrad transcript, is there anyway i could forward this copy to lsac?
