I was having trouble choosing between A and D and I don't understand the difference between those two options. They both seem to do what the passage explained "designed offal" does.

The portion of that definition that says "tailoring the production of waste from a manufacturing process" indicates that the company needs to be controlling its manufacturing process to produce waste in a certain way. All of the wrong answer choices here, including (A), can be tossed out because they do not describe a situation where a company is controlling its manufacturing process to produce waste in a certain way. For example in (A), the paper company is purchasing the recycled newspaper, not manufacturing it. Whereas in (D), the company is controlling its manufacturing of car body panels in a specific way to produce waste.
