This is a Must Be True question. The correct answer is (E).

(E) Sums up the three factors in the stimulus and infers these as a "lifestyle" that can be changed to manipulate heart disease risk.

Incorrect answers:

(A) Too strong of an inference and more of a shell game answer
(B) Stimulus doesn't address the quantity of smoking or any other factor for that matter
(C) Stimulus makes no mention of one's diet and is very exaggerated
(D) Too strong of a statement and only addresses one factor rather than all three

That's a good analysis. I would say:

(A) Stimulus says lower, not low (basically the same)
(B) same.
(C) same, more focus on no mention of diet.
(D) stimulus says one thing smoking does is influence cholesterol, doesn't say it doesn't do other things that affect the heart (this one is fairly different)

Can (A) be an answer if it states, "If a person has lower blood cholesterol, then that person's risk of fatal heart disease is lower"? I am wondering if we can infer that if higher cholesterol means a higher risk of heart disease, then lower cholesterol would mean a lower risk of heart disease.

I think that if (A) had been reworded in the way you suggest, then it could be a warranted inference based off of the second sentence of the stimulus: "The more cholesterol we have in our blood, the higher the risk that we shall die of a heart attack."
