I was wondering how to put my gpa on the application on LSAC. The schools I am applying to want you to put down your GPA at each school and I am not sure how to put my GPA because I do not know what it is. Because I went to a school, withdrew, went back, went to two schools at the same time so I do not really know what my GPA is.

Also it asks dates attended but my dates are all over the place so should I just put down my best guess?

You should have, or be able to get, copies of your transcripts from both schools, and those transcripts will provide your GPA at each school and the dates of attendance for each. Also, if you use LSAC's services to compile your applications, they will calculate your combined GPA for you and provide those transcripts to the schools for you, which should cover that requirement.
