I understand why C is right, so that all other prospects like fluctuating cholesterol levels prior to the study etc are eliminated. But doesnt D say the same thing? It provides background info on the purpose of the study, doesnt that mean its eliminating similar prospects ?

The purpose of the study is to look at the effects of a diet that includes fish on a person's cholesterol. The statement in the question " the two groups had displayed similar average cholesterol levels prior to the study" doesn't give background information about the purpose of the study, it gives background info about the participants in the study, and that is done to rule out the possibility that the two groups had different cholesterol levels before the study, which would then mean that the results of the study would not necessarily show that a diet of fish will reduce cholesterol. This leads to C as the correct answer.
